HCG Weight Loss and Diet 50 Day Medical Program

To Treat Obesity and Achieve Permanent Weight Loss

This medical program maybe purchased using the
Installment Payment Plan described below consisting of a
down payment and 12 equal monthly installment payments

Treatment period for this program is 50 days with the flexibility of
increasing the duration of the treatment period in increments of 50 days for
each additional vial of HCG (Human Chorionic Gonadotropin for Injection, USP)
10,000 Units requested by patient and approved by the Treating Physician based
upon the patient’s medical condition, weight and body mass index.

Installment Payment Plan for Purchase of This Medical Program
“Purchaser” is defined as the person identified as the holder of the credit card provided for payment in this sales transaction and the person to whom the credit card was issued.
Initial down payment amount charged to Purchaser’s credit card by American Medical General, Inc., National Medical Clinic, Inc. or any of their affiliated medical organizations (collectively referred to as “NMC”) at the time of Purchaser’s enrollment and purchase subject to the approval of the prescribed medical treatment by our physician. $999.00 Down Payment
Amount of each monthly installment payment automatically charged to purchaser’s credit card by American Medical General, Inc., National Medical Clinic, Inc. or any of their affiliated medical organizations (collectively referred to as “NMC”) during each of the twelve (12) months following the month of purchase on the first day of each month. This obligation is assignable by NMC to other organizations which shall collect the installment payments in the event of such assignments. Pay only $59.84 per month for 12 months after paying the above down payment
These monthly installment payments shall be charged to the same credit card provided by the Purchaser to pay the initial down payment to NMC at the time of sale.
The total price for the installment purchase of this medical program is twelve percent more than the posted total cost to purchase the medical program including shipping and processing fees if full payment were made by the purchaser at the time of purchase.
Refund Policy
American Medical General, Inc., or NMC, shall promptly refund to purchaser 100% of the purchase price paid for the conditional enrollment in this medical program in the event that the patient’s NMC physician declines to approve and issue to the patient the the prescribed treatment program sought by the patient after the completion of the patient’s blood test, the physician’s receipt of the patient’s blood test report, completion of the patient’s current physical examination in regard to the sought treatment and clinical evaluation of physical exam results, blood test results and medical history in relation to the patient’s medial complaint. No treatment shall be prescribed if the NMC physician determines that it could adversely affect a patient’s health or there exists no possibility of benefit to the patient from the medical weight loss and diet program treatment. This purchase is subject to the patient agreements with American Medical General, Inc. or NMC posted on this website including the “Patient Statement of Responsibility Agreement” and “Patient Waiver of Claims and Informed Consent Agreement”.
If purchaser cancels enrollment in this medical program, an enrollment cancellation fee of $750.00 shall be retained from any payments made or charged to the credit card provided by purchaser to American Medical General, Inc. or NMC and the balance of any purchase funds in excess of this amount shall be refunded to the purchaser pursuant to our policy and the patient agreement posted on this website provided that no medication has been dispensed to the purchaser by any pharmacy paid by American Medical General, Inc. or NMC for the dispensing of such medication. However, the total cost of the program purchased, or the total cost for an installment purchase if purchaser made an installment purchase (total cost of an installment purchase is an amount is equal to the sum of the initial down payment and subsequently required installment payments), whichever is the greater amount, shall be charged to patients credit card if any medication relating to this program has already been dispensed and shipped by a. pharmacy to the patient, regardless of whether or not the patient attempts to return or returns the dispensed medication. Medication cannot be returned to the pharmacy after it has been shipped to the patient or medicine packaging opened by patient. The medication shall be an approved U.S. drug manufactured by a U.S. drug manufacturer.
Authorization for Installment Payments
Purchaser agrees to fully comply with the terms and conditions of the Installment Payment Plan posted on this website for this purchased medical program. Purchaser agrees to make the down payment specified above for this medical program and authorizes American Medical General, Inc., or its affiliate, to make a recurring automated charge of the monthly payment amount to Purchaser's credit card provided by Purchaser at the time of sale for the next twelve (12) months on the first date of each month. In consideration of this agreement, Purchaser waives all defenses and claims in the event purchaser fails to make any of the agreed 12 monthly payments and stipulates to an entry of a default judgment against Purchaser for the unpaid amount owing on this installment Purchase Plan plus collection fees and reasonable attorney's fees and costs incurred by American Medical General, Inc., its agents or assigns, in any act to collect the payments due or in regard to legal action filed against Purchaser to recover the unpaid balance owing on this installment payment plan. Interest shall accrue on the unpaid amount due in the event of Purchaser’s failure to pay one or more payments due at an Annual rate of thirty-three percent of the amount owing.
